What Chemistry Control Level 1 does

Control material has independently assigned target values and is run to answer a question rather than to adjust the instrument: does this analyzer, today, still produce the result it should? Level 1 sits within the normal range for most analytes, which makes it the level that reflects the concentrations most patient samples arrive at. It is run alongside level 2 rather than instead of it, because a control in the normal range can look entirely acceptable while the analyzer is meaningfully wrong at the concentrations where clinical decisions are made. The correct response to a failed control is to reject the run and investigate, never to recalibrate until the control agrees.

How Philippine laboratories run it

Run at the frequency set by the laboratory quality control policy, which is typically at least once per day of testing and after every recalibration, and recorded so that the history can be shown to an accreditation assessor. Philippine laboratories running more than one shift consume control material considerably faster than a single-shift site at the same test volume, and this is the calculation most often got wrong when planning supply. Questions about Chemistry Control Level 1

Why run two levels of control instead of one?

Because an analyzer can be accurate in the middle of its range and wrong at the extremes, and the extremes are where clinical decisions get made. A single normal-range control would look perfectly acceptable while a patient with a genuinely high or low result was being reported incorrectly. Two levels bracket the range that matters.

What should we do when a control fails?

Reject the run and investigate before releasing any patient results from it. The one thing not to do is recalibrate until the control comes into line, because that hides the problem rather than finding it. Check the obvious causes first, including reagent condition, control handling and recent maintenance, then repeat before deciding.
