How a coagulation reagent works

A coagulation analyzer does not measure a concentration. It measures a time. Citrated plasma is mixed with a reagent that starts the clotting cascade at a chosen point, and the instrument watches the sample until a clot forms, reporting how many seconds that took. Which reagent is added decides which part of the cascade is being interrogated, so two tests together localise where a prolonged result is coming from rather than merely reporting that something is wrong. Because the result is a time and not an amount, everything that changes the timing changes the result, which is why the pre-analytical requirements on this bench are stricter than anywhere else in the laboratory. The tube has to be filled to the mark, because the citrate-to-plasma ratio is part of the chemistry rather than a formality, and it has to be run within the stated window. A short draw or a haemolysed sample produces a plausible number that is simply wrong, and nothing downstream will flag it.

Why does the citrate tube have to be filled exactly to the line?

The anticoagulant in the tube is measured against a full draw. Underfill it and the plasma carries proportionally more citrate, which extends the clotting time and produces a falsely prolonged result. This is the most common preventable error on a coagulation bench in any laboratory, and no analyzer can detect it after the fact, so it has to be caught at the point of collection.
