What Phosphorus measures
Phosphate is regulated jointly with calcium by parathyroid hormone and vitamin D, and the two are read as a pair because their relationship is more informative than either value alone. In chronic kidney disease the failing kidney retains phosphate while calcium tends to fall, and that reciprocal movement drives the secondary hyperparathyroidism and the bone disease that follow. Phosphate is also measured in the investigation of rickets and osteomalacia, in parathyroid disorders, and in patients being refed after prolonged malnutrition, where a sharp fall is anticipated and dangerous. It is one of the analytes where the trend across serial measurements says considerably more than a single value.

Why are calcium and phosphorus always ordered together?
Because the same hormonal system regulates both, so their relationship carries information neither value holds alone. In chronic kidney disease phosphate rises while calcium falls, and that opposing movement is the finding that prompts treatment. Measuring one without the other loses the pattern and leaves a result that is difficult to act on.
Why is phosphate followed closely in a malnourished patient being fed again?
Because reintroducing nutrition drives phosphate out of the circulation and into cells rapidly, and the level can fall far enough to be dangerous within a day or two of feeding starting. Units managing that risk measure it before feeding and then at short intervals, so a single patient generates repeat requests and the laboratory needs the assay available rather than batched.
