What Creatinine measures

Creatinine is produced by muscle at a fairly constant rate and cleared almost entirely by the kidneys, and that steadiness is what makes it the more specific of the two renal markers. Because production depends on muscle mass, the same concentration means different things in a large young adult and a frail elderly patient, which is why laboratories report an estimated glomerular filtration rate calculated from the creatinine alongside the raw value. The relationship is not linear: filtration can fall substantially before the creatinine leaves its reference interval, so a result at the upper end of normal in an older patient may already represent meaningful impairment. Creatinine is also used to stage chronic kidney disease and to adjust the dosing of drugs cleared by the kidney.

What does the SOX in the Mindray CREA (SOX) product name mean?

It identifies the method: sarcosine oxidase, an enzymatic determination of creatinine. It is worth knowing which method your laboratory runs, because the older Jaffe chemistry and enzymatic methods do not always agree closely, and eGFR equations are derived against standardised creatinine values. If you are comparing results with another laboratory, the method is part of the explanation for any difference.

Can a normal creatinine still mean reduced kidney function?

Yes, and this is the assay’s main limitation. The relationship between creatinine and filtration rate is not linear, so filtration can decline substantially before the creatinine rises above the reference interval, particularly in patients with low muscle mass. That is why laboratories report an estimated GFR calculated from the creatinine rather than leaving the raw value to be interpreted alone.
