What Total Protein measures

Total protein measures albumin and the globulins together as one figure, which on its own is a fairly blunt instrument: a normal total can hide a low albumin offset by raised globulins. Its value comes from being paired with a separate albumin measurement, because subtracting one from the other gives the globulin fraction and with it the albumin-globulin ratio. That ratio is what shifts in chronic liver disease, in chronic infection and in conditions that drive immunoglobulin production, often before either individual value leaves its reference interval. The laboratory measures two analytes and reports three numbers, and the third is generally the one that carries the clinical signal.

How Philippine laboratories run it

Philippine laboratories almost always report total protein, albumin and the calculated A/G ratio as a block, and clinicians ask for the ratio by name. Because the globulin figure is calculated rather than measured, there is no separate globulin reagent to order: the supply plan needs total protein and albumin in step with each other, and a laboratory that runs out of one has effectively lost the ratio as well. Questions about Total Protein

Is there a separate reagent for globulin?

No, and there does not need to be. Globulin is calculated by subtracting the measured albumin from the measured total protein, so the two reagents above produce three reported values between them. This is why total protein and albumin should always be ordered on the same schedule: running out of either one takes the A/G ratio with it.
