What HDL-C measures
High-density lipoprotein carries cholesterol away from the tissues and back to the liver, and higher concentrations are associated with lower cardiovascular risk, which reverses the usual reading of a lipid result. It is measured directly on the analyzer using a reagent system that selectively makes the cholesterol in the HDL fraction available to the enzymatic reaction while leaving the other lipoproteins alone, which is why this reagent is chemically more involved than the total cholesterol reagent it accompanies. The measured HDL is also an input to the calculated LDL where a direct LDL is not run, and it is used in the cholesterol-to-HDL ratio that many risk assessments report.

Questions about HDL-C
Is HDL cholesterol measured or calculated?
On the Mindray BS series it is measured directly, using a reagent system that makes only the HDL fraction available to the enzymatic reaction. That is a meaningful difference from LDL, which many laboratories still calculate, because a directly measured value stays valid when triglycerides are raised whereas a calculated one does not.
What is the cholesterol to HDL ratio, and do we need a reagent for it?
No separate reagent is needed. The ratio is calculated by dividing the measured total cholesterol by the measured HDL, so the two assays already on the bench produce it. Many Philippine reports include it alongside the individual values because it expresses in one number how much of the total sits in the protective fraction.
