What Lipase measures

Lipase breaks down dietary fat and comes almost entirely from the pancreas, which gives it two clear advantages over amylase in suspected pancreatitis. It is not raised by salivary gland disease, so it produces fewer misleading elevations from a source outside the abdomen, and it stays elevated for several days after the onset of illness rather than falling back within a couple of them. A patient who reaches the laboratory late, which is common, may therefore have a normal amylase and a clearly raised lipase. The two are ordered together where both are available, and lipase is generally the one given more weight when they disagree.

How Philippine laboratories run it

Lipase runs at noticeably lower volume than amylase in Philippine laboratories, because many still offer amylase as the routine pancreatic test and add lipase only when specifically requested. That gap is worth reviewing rather than simply supplying, since the clinical case for reporting both is strong and the late-presentation scenario the lipase covers is common in practice. Laboratories deciding whether to add it should look at how many of their pancreatitis requests arrive more than forty-eight hours after the onset of pain.

Should we report lipase as well as amylase?

Where you can, yes. Lipase is more specific to the pancreas and stays raised for several days, so it catches patients who present late and whose amylase has already returned to normal. Reporting both gives the clinician a more reliable picture than either alone, and where the two disagree the lipase is generally the more informative result.
