What hs-cTnI measures
Cardiac troponin I is a protein of the heart muscle contractile apparatus, released into the circulation when cardiac myocytes are injured. High-sensitivity assays measure it at much lower concentrations than earlier generations could, which allows the change between two samples taken a defined interval apart to be assessed rather than only a single value. That serial change is central to how the result is used. Troponin is reported as part of the chest-pain workup, and the cut-offs, sampling intervals and interpretation are the responsibility of the laboratory and the treating clinician. Raised troponin indicates myocardial injury without specifying its cause, and a number of non-coronary conditions raise it.

This assay is defined operationally by the emergency department rather than by the laboratory. Requests arrive at any hour, they are urgent, and they come in pairs or series on the same patient because the change between samples is what is being read. Why are two troponin samples taken rather than one?
Because a high-sensitivity assay detects troponin at concentrations where the change between two timed samples is more informative than a single value. The sampling interval and the criteria applied to that change are set by the laboratory and the treating service. Interpretation of the result, and any decision that follows from it, belongs to the clinician rather than to the assay.
Does a raised troponin always mean a heart attack?
No. Troponin indicates injury to heart muscle, not the reason for it. Renal impairment, sepsis, arrhythmia, heart failure, pulmonary embolism and several other conditions can raise it without coronary occlusion. That is why the result is reported as part of a workup and read alongside the clinical presentation and the electrocardiogram rather than on its own.
